What ACHIEVE-3 tested and who participated
ACHIEVE-3 was a 52-week randomized controlled trial comparing orforglipron (now Foundayo) and oral in adults with that was inadequately controlled on existing therapy.
The trial compared two dose pairs: orforglipron 12 mg versus oral semaglutide 7 mg, and orforglipron 36 mg versus oral semaglutide 14 mg. These are the maximum approved doses for each drug in the T2D setting. The primary endpoint was non-inferiority of orforglipron to oral semaglutide — the trial was designed first to ask whether orforglipron was at least as good. Superiority was tested only after non-inferiority was established.
Participants were adults with type 2 diabetes, inadequately controlled on existing therapy. This is an important framing point: the trial population had T2D. People without diabetes who take for weight management are a different population.
What the numbers show
On the primary endpoint, orforglipron not only met non-inferiority but demonstrated statistical superiority at both dose comparisons.
Mean reductions:
- Orforglipron 12 mg: –1.9% versus oral semaglutide 7 mg: –1.2%
- Orforglipron 36 mg: –2.2% versus oral semaglutide 14 mg: –1.4%
Weight loss followed a similar pattern. In the higher-dose comparison, Foundayo achieved a mean weight reduction of approximately 19.7 lbs versus approximately 11.0 lbs for oral semaglutide at 52 weeks.
Lilly's press release reported 57.1% greater relative A1C reduction and 73.6% greater relative weight loss at the highest dose comparison. The relative numbers are mathematically correct but reflect the starting denominators; the absolute differences (1.4 percentage points in A1C; approximately 8.7 lbs in weight) are the clinically meaningful figures.
What "superiority" means in trial language
A clinical trial that meets a superiority endpoint has shown a statistically significant difference in the pre-specified outcome between the two treatments. This does not mean the superior drug is "better" for every patient, or that the differences are clinically meaningful for every individual.
In ACHIEVE-3, the A1C differences between the two oral GLP-1s are meaningful on a population level — but individual response varies. Tolerability, dosing schedule, cost, and coverage matter alongside efficacy averages.
The practical differences between these two oral GLP-1s
Dosing restrictions: Oral semaglutide requires specific dosing conditions — the tablet must be taken on an empty stomach with up to 4 oz of plain water, and you must wait at least 30 minutes before eating, drinking anything else, or taking other medications. This restriction exists because oral semaglutide's absorption depends on specific gastric conditions.
Foundayo has no food or water restrictions. It can be taken with or without food and at any time of day. This difference may affect adherence and tolerability in practice.
Approval status and : Foundayo was approved by the FDA in April 2026 for type 2 diabetes and cardiovascular risk reduction. The Wegovy tablet was approved for weight management in people with or overweight with comorbidities. These are different indications. ACHIEVE-3 was conducted in a T2D population and evaluated the T2D indication.
What ACHIEVE-3 does not tell us

What remains uncertain
Foundayo's obesity/weight management indication was pending as of the time of this writing (July 2026). If and when that indication is approved, comparative efficacy for weight loss in people without T2D will be more directly relevant.
