Key takeaways
- List prices for branded injectable GLP-1 medications are approximately $1,300–$1,400 per month; most patients with coverage or savings program access pay substantially less
- With commercial insurance and a manufacturer savings card, some patients pay as little as $25 per month
- Without commercial coverage, Novo Nordisk's NovoCare Pharmacy offers Wegovy at $349 per month for standard doses
- Medicare Part D beneficiaries can access certain GLP-1s for $50 per month through the Medicare GLP-1 Bridge starting July 1, 2026
- Oral GLP-1 options may offer different entry-point costs depending on coverage
- Compounded GLP-1s are no longer broadly available through legal channels following FDA enforcement — they should not be assumed as a default low-cost alternative
List price vs what you actually pay
The wholesale acquisition cost (list price) for branded injectable is approximately $1,300–$1,400 per month. This is the number that often appears in news coverage and may appear on a pharmacy receipt before insurance adjustment.
Few patients pay this amount. What you pay depends on which of the following situations applies.
If you have commercial insurance with GLP-1 coverage
If your employer plan covers GLP-1 medications for weight management, your co-pay is determined by your plan's formulary tier. After applying a manufacturer savings card, patients with commercial coverage may pay as little as $25 per month for (subject to NovoCare savings program eligibility and terms). Lilly's LillyDirect program has comparable savings options for .
NovoCare and LillyDirect savings programs are subject to change. Verify current eligibility and pricing at novocare.com and lillydirect.com — terms vary and are updated regularly.
If your commercial insurance doesn't cover GLP-1s for weight loss
If your employer plan has dropped GLP-1 coverage, or never offered it, the manufacturer pharmacy programs provide the most accessible self-pay option.
NovoCare Pharmacy (Novo Nordisk's direct pharmacy) offers Wegovy injection at the following prices as of June 2026:
- $349 per month for standard injection doses (0.25 mg, 0.5 mg, 1 mg, 1.7 mg, 2.4 mg)
- $399 per month for Wegovy HD (7.2 mg)
- $149 per month for new patients starting at lower doses (1.5 mg and 4 mg) through the NovoCare Pharmacy program
These are program prices, not list prices. Eligibility requirements apply. The NovoCare program has time-limited offers (the $149 pricing applies through certain dates — verify current terms at novocare.com).
For Zepbound (tirzepatide), verify current self-pay pricing through LillyDirect at lillydirect.com.
If you are on Medicare
Starting July 1, 2026, eligible Medicare Part D beneficiaries can access certain GLP-1 medications for $50 per month through the Medicare GLP-1 Bridge — a short-term CMS demonstration running through December 31, 2027.
Before this program existed, GLP-1 coverage was largely absent from original Medicare Part D, leaving most Medicare beneficiaries to pay full cost or use manufacturer programs. At $50 per month, the Bridge represents a meaningful shift for this population.
Eligibility criteria and the specific covered drug list are on the CMS Medicare GLP-1 Bridge page (cms.gov). Verify with your Part D plan before July 1 to confirm your medication is included and whether any is required.
Oral GLP-1 options and their pricing
Two oral GLP-1 medications are now for weight management:
Wegovy tablet (oral semaglutide, 25 mg once daily). Approved December 2025. Requires taking on an empty stomach with a small amount of water, waiting 30 minutes before food. With commercial coverage, some patients may pay $25 per month or less through savings programs (verify with NovoCare).
Foundayo (orforglipron, once daily). Approved April 1, 2026. No food or water restrictions. With commercial insurance coverage, starting costs have been reported at approximately $25 per month. Verify current pricing and coverage through your insurer or LillyDirect.
Oral GLP-1 pricing outside of commercial coverage is still establishing itself. If you are considering switching from an injectable to an oral option for cost reasons, verify the actual out-of-pocket difference under your specific plan before making a clinical decision.
Compounded GLP-1s: what's still available and what to know
During the 2022–2025 shortage, semaglutide was widely available through telehealth platforms at prices often well below branded options. That access path has largely closed.
The FDA removed semaglutide and tirzepatide from the in 2025. Following that, FDA restricted compounding to a maximum of four prescriptions per calendar month for essentially-a-copy formulations, and proposed to permanently exclude semaglutide, tirzepatide, and liraglutide from the 503B outsourcing facility Bulks List. Over 455 adverse events linked to compounded semaglutide have been reported to FDA.
If a platform is still advertising compounded semaglutide at low prices, verify the legal and quality basis for that claim before proceeding. Compounded GLP-1s at scale are no longer the broadly accessible alternative they were in 2023–2024.
What remains uncertain
Pricing programs change. What you pay in July 2026 may not be what you pay in early 2027. Savings card terms, coverage decisions, and program availability are all subject to change. New GLP-1 drugs entering the market may alter the competitive pricing landscape. Oral GLP-1 pricing at scale outside commercial coverage is still establishing itself.
Questions to ask your prescriber or pharmacist
- Is my specific GLP-1 covered under my current plan, and what is my co-pay?
- Am I eligible for the NovoCare or LillyDirect savings programs for my medication?
- Do I qualify for the Medicare GLP-1 Bridge starting July 1?
- Is an oral GLP-1 option appropriate clinically, and what would it cost under my coverage?
- If my coverage changes, what is the transition plan for staying on medication?
